2) This is a public forum, of almost exclusively fellow software users.  It IS NOT ProSeries support.  No one from here will be emailing you.

3) Post exactly what you've tried and some fellow PS user might be able to help (I'm a Lacertian..).

4) FWIW - the state forms aren't ready for prime time yet if you are actually asking why the state forms aren't appearing.
