Pretty sure the exclamation column is if you use the missing info feature and the 4th one shows locked returns. Not sure, nor can I read the what the middle 2 columns are for. One I can't read at all and the other looks like 99?

I have a different column order but can you just grab the column and stretch it?
My order of first four columns are
1. !
2. lock icon
3. file name
4. type

Can someone tell me what the columns mean? One is unreadable and the other is 99 or quotation marks?

the other column looks like a pencil. maybe returns with a mark on the respective file's row would mean there are notes on that return. I never realy even noticed those four columns.. looks like they appear in the EF Center
