Is it narcissism when someone thinks highlighting is done only for them and not for the person who may want to look at the document again in a year or two and wonder what it means?  No, I don't think that's narcissism.  But as always I agree with @PATAX that there is a plausible explanation for this that does not involve helping another person who might look at the document and then, presumably, return it.
