You didn't include which version of ProSeries you're using, but in Professional, in a regular HomeBase view you can customize the headings to include a DOB, then print your list.

Top menu bar   Homebase > Column Headings

Remove the ones you don't want and add the ones you do wants and create a custom HB view.
