1099R only has boxes 3-10 on form. Box 3  employee contributions box 4 contributory amts, box 5,6,8,10 with no amounts, box 7 totals, box 9 Fed Tax W/H, box 10. Help?

At the very top of 1099R input screen check the far right box RRB-1099R.  Then your eyes will be enlightened with more choices to enter boxes 3 and 4 etc.
