Not sure why the database isn't updating to reflect that. As a test, try adding a column to your client screen for 4868 to see if it registers there. It'll have a check in the row for any client that has been put on extension. Go to Options, Display tab. On the left open the section for Forms and Schedules, select 4868 and move it to the right column.
