There won't BE a F 1099-R for 2021.  The entire distribution was shown on the 2020 F 1099-R 'cuz that's when the funds were distributed. 

The election to spread the taxability over three years was should have been done on the 2020 tax return & would have been shown on F 8915-E (I think... I didn't have any of these).
