Client last year was starting to suffer from dementia. This year, he is way worse. He asked me to file his taxes this year based on last year's tax returns. (BTW...I am not trained in this malady so I can't make a diagnosis.)
I don't think I should do that; thou I think I'll get very close. Says he can't find any source documents.
What to do?

Form 4868.
Then ask him for the names of family or friends you can contact to ask for help. If none, contact your jurisdiction's agency for adult protective services.
Or depending on his resources and attitude, he might be convinced to hire a licensed private fiduciary (which might be what he expects of you, but you are not trained or licensed for that). You might volunteer to help him find one.

Put him on extension. Later his authorized representative can file to get his tax transcript. You are not responsible.
