The taxpayer made several attempts to get the RMD from his 403B account with Fidelity and kept hitting brick walls. Finally rolled entire account over and had new company issue RMD, but it is dated 2023.

File the 5329 with the 2022 1040 asking for a waiver of the penalty for not taking the RMD.
The distribution received in 2023 goes on the 2023 return.
The more I know the more I don’t know.
