The IRS tells you they will not process an efile extension that itself is late. Look here:

Extensions were due on 4.18.22. One doesn't get to request an extension *after* the due date.
My suggestion to your tardy client is to get the return filed ASAP, as well as get any taxes owed paid too.
IF there's a refund... the impact of a late return isn't generally severe (baring any elections that could have only been made on a timely filed return.
