The instructions & Pub don't specify if the value of a rollover IRA account should be included in line 6, although it says "all" traditional IRA's.

All IRAs really does mean all IRAs. Except Inherited IRAs still titled in the name of the deceased with the taxpayer listed as beneficiary.
