Client received a distribution from a Canadian Insurance Company, named Sun Life. They withheld Canadian tax. Client is a US Resident that has not lived in Canada since 1977. Looking for the instructions and tax form I need to report this distribution to Canada, to determine if we owe additional tax or if a refund is due.

That was no help. I have spent the better part of three days just trying to talk to someone at Canada Revenue. I am looking for someone to tell me the publication and the form. I just can't find it on their website

Sun life has materials available on line that may give you an answer to this.
Most likely you would file as income and report foreign tax paid for a credit against US tax.
