Should the amount shown in Box 13 1099-INT be entered on the Interest Worksheet be section D (4) or E (t)?

Both will achieve the same result. I would, however, use D4 for what's reported in Box 13 for covered securities and reserve E(T) for noncovered securities where amortization would have to be computed offline.
