Where do you tag the T or S in the interest & dividend income sections?
Right on the line you are entering the interst or dividend income there is a faint letter T that shows up by defualt. click on it to change it.

I have an Ohio question. The software doesn't seem to have the City of Toledo? I've had to do these online for my clients.
Do you have the T, S or J Indicators checked in the Tools Options section?
