as far as I can remember it is supposed to be paid for by the retired employee. it doesn't show up on the 1099-R form but if you get their quarterly pension statement you can see the amounts paid each month for the medical insurance. 

as for where the input:

1099-r worksheet scroll down to about mid page and you will see INSURANCE

it is the 3rd line under insurance
