Are you sure you mean F8833 -  Treaty-Based Return Position Disclosure ?

If you are, in fact, referring to F.8833 and assuming your client is filing MFJ, both the taxpayer's and spouse's names should appear on the form.  If there is anything particular you would like to highlight, you could include that as part of Line 6.
