Client had a nanny share arrangement with another couple. Cash was paid, no W-2 issued. Can I prepare the return omitting the nanny or would ethical standards prohibit me?

Ethical standards would prohibit. The nanny should get a W-2 - or 2 (one from each couple)

How much was paid to the nanny? Don't you have to exceed a certain amount before a W2 needs to be issued?

Thinking “self prepared” will get you off the hook is like saying you can shoot someone and not get in trouble as long as the gun wasn’t registered to you.
