Although I am not clear on why checking one precludes the other?  If I uncheck the surviving spouse and check the QTIP election then page 2 of 706 in box 4 shows none and instead shows her in box 5 as someone other than the surviving spouse.  Seems like I am still missing a step here to get Sch M part A to populate correctly.
