How to Generate Form FTB 3567 for California Installment Agreement

To generate the Form FTB 3567:

1. In the Detail input, go to Screen 80, Installment Agreement Request.
2. Fill out the information under the section California Installment Agreement Request (3567).

Per the California Franchise Tax Board (FTB) website, the Installment Agreement Request may be filed in one of 3 ways:

• Online via the Installment Agreement - Apply Online page on the FTB website
• Via phone using the Interactive Voice Response system at (800) 689-4776
• By completing and mailing form FTB 3567 through the USPS
