Is the total contribution in box 2 of form 5498-SA includes the employer and employee contributions to the FSA ?
If this is a cafeteria plan should the amount in box 2 of form 5498-SA be the same amount that is stated in box 12 of the W-2 with code W?

You don't do anything with the 5498. You enter the W2 info into the taxpayer's return.
