Based on the attached sale of short and long term securities, was the information entered correctly on schedule D ?

There were many more short-term transactions but I am more concerned about the one that had the wash sale disallowed on both, the short-term and long-term transactions. Please see attached. Thanks.

Just use capital gains/loss worksheet, and make two entries short term covered, and long term covered. Put total wash sales in proper box. for covered transactions you don't need the detail
