I have a small non-profit that pays for sensitive medical costs. Do the donee's names and address have to be on sch O for grants given in excess of $5000? Can we use Family 1 instead of a name to protect their privacy?  Or attach a manual schedule to show the details? Feel like there is a HIPPA issue here. Thank you.
