Like if a client has early stages of dementia or other health issues. Something we can have the client sign stating we have permission to speak with a certain person about their tax information. We have some elderly clients that get confused easily as well. Sorry, I have never done this before.

Hopefully the client has a durable power of attorney. This allows someone to make medical decisions.
Do they have a living trust? If so, it generally will name a trustee to take over when the client is unable to make their own decisions. Talk with the client and find out who they trust to take care of things and encourage them to get in touch with an attorney to get things in order. Possibly have the court assign a conservator.

@abctax55 suggests 2848 so someone can actually sign returns and act in their place for tax matters.

Are you looking for something like the organizer? It lists the items from last year, with a slot for the number for this year? It is printed from the prior year program, so if you have the 2017 Lacerte, you would print from that for gathering the 2018 information.
