Form 1040 X. If client has not yet received refund and now refund is lower due to withhoding adjustment do I report original refund not received on Form 1040X ? If I do it appears the taxpayer has a balance due and not a smaller refund

Yes, they have a balance due. They can wait for the refund to show up, but the interest due on the balance due will increase.
