Do I have to file the entire NY return if the client only needs the NYC 210?. When I went to the e-file tab, and then to e-file special forms, it is not letting me choose just the NYC 210. Is there a way just to file this as a stand alone form, or I have to file the whole return?

It would appear that https://www.tax.ny.gov/pit/ads/efile_addnyc210.htm is the only way to go, if that is all you want to do.
Personally, I like to file 1040, even if not required. That helps to assure that the SSN is not used by someone else to file a fraudulent tax return.
