Reporting tax and fuel credits on Form 4136 in ProSeries

To generate Form 4136 on an Individual return:

1. Press F6 to bring up Open Forms.
2. Type 4136 to highlight the 4136.
3. Select OK to open the form.
4. Complete the required information.
5. Complete the new Statement Supporting Fuel Tax Credit (FTC) Computation - 1.
6. Print the new statement to PDF. For instructions on printing a selected form to PDF see here.
7. For e-filed returns, add the completed PDF via PDF Attachments. For instructions on adding the PDF attachment see here.

For Corporate 1120 filers:

1. Press F6 to bring up Open Forms.
2. Type 4136 to highlight the 4136.
3. Select OK to open the form.
4. Complete the required information.
