Why are the carryforwards of prior years credit and information from form 5695 not carrying forward on the 2021 Form 5695?

The 5695 with the carryforward carries over for me fine. I dont see any carryover worksheet that tracks the 5695 carryover, expect the 5695 itself.

Did you transfer that client file into 2021 early in the season?  Sometimes early transfers dont grab everything.
