Where do I report foreign retirement income of a US citizen?

US citizen worked in Germany for several years.  Returned to US and was gainfully employed until she retired last year.  She received no retirement form but funds were deposited into her bank account from Germany.  She also receives Social Security.  What form do I muse to report the distribution of the retirement from Germany?

What is the income from?  German Social Security?  A pension?  The German equivalent of a 401k or IRA?  Something else?
