What is the file name for Rates per Form?

I had to reinstall PS 19. I have the old directory and files.  All my prices are the old ones. I need to know the name of the file that holds the rates per form to move it to the new install so I don't have to go and change all the prices again.

I dont use rates Per Form, but when I do a new install, I use the COMMON folder from the old computer to replace the COMMON folder on the new install and that seems to bring all my settings over.
