What form(s) to report a foreign pension?

Client received a foreign pension statement. what form(s) do I use to report the income?

An exchange rate is provided on the statement for the US amount as Euro. The Euro rate used to determine the US amount, is not on the statement, do I need to know it?

What do I use for the tax ID number to e-file the return?

Any assistance that can provided is appreciated.

Convert to the appropriate amount in US Dollars and report as "Other Income" = "Foreign Pension".
