W-2 and 8919 form , uncollected social security and Medicare tax

I am new in ProSeries. Client has a W-2 that box 3,4,5 and 6 have no numbers, but have federal income tax withheld and state income tax. I made a 8919 form for uncollected SS and Medicare tax but I can not find any box to enter Federal and state income tax. However, the income added to line 1 in 1040 with this statement :  F8919.

If I enter all data of W-2 separately in ProSeries, the amount of line 1 in 1040 will be double. I am confused what should I do with this W-2 for tax withholds parts, Fed and state?

It is quite possible the W-2 is correct, so don't file 8919 until you know more.  Generally 8919 is used when a person received a 1099-NEC but it should have been on a W-2.

Is this a minister/clergy?

Thanks, but I have read 8919 instruction that can be for w2.

Client for last 3 years had 8919  as same what I am going to do for 2021. I have seen client 2020 tax return and her W-2 with 8919 which it was filed.

As I said before, you need to find out more.  In some cases, the W-2 may be correct when it doesn't show any Social Security or Medicare wages.  Just because the prior preparer filed 8919 does not necessarily mean that is correct.

Find out exactly what type of work this person does for the W-2 and who is paying him.  And is he a student?  Nonresident Alien?  Minister?  Household Employee?  Some other special circumstance?
