Transfer of 3rd party designee- Part V of the Federal Information Worksheet box is checked. Pd preparer should transfer to pg 6 and the yes box should be marked.

The paid preparer does not get listed on Schedule 6.
From the 1040 Instructions:
If you want your paid preparer to be your third party designee, check the "3rd Party Designee" box on page 1 of Form 1040. Do not complete Schedule 6.
