I can not get the system to print the taxpayer name and EIN on the second page of form 2848 ( POA)

does anyone know why system is not printing name and EIN on the 2nd page of form 2848

2848 for what type of entity?ptrshp? corp? trust?

I don’t see a place to enter the EIN on the second page of the 2848.  There is a place under the taxpayer signature line to print the name of the person who signs -- for entities that are not individuals, you may not even know the name of the person signing until they fill out their title.
