How do I notify IRS that I am filing a married filing separate return which was based on a married filing joint return extension? The extension was e-filed.

I don't think you do, at least not right now.
Inform your client that there might be questions down the road, if the IRS fails to notice the second SSN on the approved extension. And get ready to answer any inquiry that comes later on.....
