For a 1099-R, the taxable amount is being included in Line 4b, but the total amount is not included in 4a. Taxable (4b) shows as more than 4a. Is there some reason?

I have checked all input.  This client has both IRAs and pensions.  Some of the IRAs have the taxable amount listed and some don't, but for all of them the IRA amount is not included in 4a.  Is this a bug, or something I don't understand?

The 1040 instructions say to not show an amount in 4a for IRAs that are fully taxable.
