Arizona Form AZ-8879

Client's return shows they need to sign the E-file Authorization form for Arizona.  AZ-8879.
The form is no where to be found in Pro-Series.  I've also been looking through the Arizona Dept of Revenue/Taxation website and cannot find one there.  
Any suggestions?

You sure you have AZ set for Efiling in the Fed Info worksheet?

I see it in my AZ list of forms in use.
