8821-S spouse not printing correctly

There is a printing problem on Form 8821.  While 8821-T prints correctly.  Take the Form 8821-S and fully populate it.  When it prints, it fills in the client's name and SSN but not anything below.

The rep fields are blank and all other info on the form.

    It also does not show up in the Select Forms to Print popup.  Thus you cannot print the form in Select mode.
