Is it mandatory/Are we required to enter our clients Driver's License, issue date, etc under tax authentication?

1.  Does IRS require that we ask our clients for proper identification such as driver's license id card, etc.?  

AND 

2. Are we required/or is it mandatory for us to enter ALL this information (Drivers license number, issue date, expiration date, etc;) in Screen 1 under Tax Authentication?

Just a note, the Federal requires SOME response to be entered, even if you say no driver's license was shown to you.

The IRS doesn't require it, but there are a few states that do.
