Can a check box be added for form 5405 when it is not required? I have a client that gets an IRS notice . A statement in the margin "Not required to file Form 5405"

If it wasn't required, you normally wouldn't have entered sufficient details for Lacerte to generate the form. How was the form generated if your client had no filing requirement and did you not have any diagnostic?
